Health and Wellness

An annual physical examination including routine labs is included in your membership. Personalized coaching to create positive lifestyle changes such as weight loss, smoking cessation, and stress reduction are provided. An initial referral to a certified nutritionist and a personal trainer will be offered.

Immediate Medical Care

Same day or next-day services for immediate medical needs such as minor lacerations, infections, sprains, fevers, etc. If you are not sure if your condition is something we can take care of, give us a call and we will direct you to the appropriate facility to address the problem.

Chronic Condition Management

We will manage chronic conditions such as high blood pressure, diabetes, high cholesterol, asthma, arthritis, depression and others. We will provide necessary medical care as well as empower you to  manage your own health keeping up with current guidelines. We will understand your individual situation and work to maintain stability of the chronic condition. If complications or concerns arise you will be referred to a qualified specialist.

Specialist Care Coordination

We will coordinate your care through communication with your specialist and work with you to make decisions regarding follow through with your care plan. As your primary care provider we should be involved in any specialist care you seek and ask that you contact us to make sure the referral is appropriately coordinated.

Hospital and Emergency Room Visit Coordination

In case of  serious illness requiring emergency or hospital care, we will communicate with the hospital, send relevant records, and coordinate your care during and after discharge. In case of an emergency, please call 911 and then have the hospital personnel call us so we can participate in your care. We will keep in close contact with the hospitalists who are providing direct care to you and we are available for concerns and questions at any time.

Laboratory Tests

We offer basic screening tests on site such as pregnancy, strep throat, urinalysis, etc. Other tests may be sent to our partner laboratory who can bill your insurance directly or you may take advantage of the low costs of cash pricing which can be up to 75% of regular pricing. All testing and associated costs will be discussed with you prior to ordering and processing those tests.

Medications

Prescriptions can be sent to the pharmacy of your choice. We will discuss the best options for appropriate therapy and costs prior to the prescriptions being sent.

Vaccines

We do not offer routine vaccinations in this office.  We recommend the local health department which can provide immunizations at a reduced cost.
